Insurance Information

Pain Consultants of Florida accepts most insurance plans including HMO, PPO, POS, Workers Compensation, Auto Insurance and Medicare. 

Authorizations

Most HMO's and some PPO's require prior authorization before your initial consult, follow-up office visits and prior to any injection or procedure that may be recommended.

 

  • Office Visits: Patients need to obtain authorizations for each visit, usually from their primary care physician. The authorization should be valid for date of visit, location, provider and level of service.
  • Procedures or Injections: We will obtain authorization based on the codes being used. For Medicare patients no prior authorization is required. If a procedure is recommednded that may not covered by Medicare a patient will be asked to sign an advanced beneficiaries notice (ABN) explaining payment options if Medicare denies the service.

 

Referrals

We see patients by referral only (no walk in, self referrals, etc.). Patients are typically referred by another physician, work comp case manager, attorney, or insurance company.
